If you require any vaccinations relating to foreign travel you need to complete our travel risk assessment form – This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

A Form should be filled in of your holiday destination and an appointment made 4 to 6 weeks before travelling.

  1. Complete and return Travel Risk Assessment Form before coming to see the nurse.

  2. Attend appointment with the practice nurse to discuss travel arrangements and receive your vaccinations.

  3. If you do not need any vaccinations or malarial medication, the nurse will contact you.

Some travel vaccines incur a charge – this is because not all travel vaccinations are included in the services provided by the NHS.
