New Patients

New Patient Notice : We are unable to accept registrations from outside of our catchment area. Please click HERE to check whether your home falls within our catchment area prior to filling out the application form. 

If you live within our practice area you may register by completing a standard new patient registration form and providing proof of identity – photo ID and proof of address – for example Passport, Birth Certificate, Driving Licence, utilities bill. Once registered your records will transfer from your current GP practice. You are required to complete a New Patient Health Questionnaire and the details from this will be recorded on your records.

Named Accountable GP

When you register with the practice you will be given a named accountable GP who is responsible for your overall care at the practice. This does not prevent you from seeing any GP at the practice that you wish. You should contact the practice if you wish to know who this is, and if you have a preference as to which GP that is, we will make reasonable efforts to accommodate this request.

Armed Forces

Since 1 April 2015, GMS Regulations allow for a serving member of the armed forces of the Crown, who has received written authorisation from Defence Medical Services (DMS), to register with a GP in whose practice area they live or work during the period in respect of which that written authorisation is given, for a period of up to two years.

In these circumstances, DMS will retain responsibility for meeting occupational health needs, but the individual’s primary care needs would be delivered through registration for NHS primary medical services with a GP practice.

Veterans

When you leave the Armed Forces, you will need to register with your local GP. You should make sure that the GP has all the information they need about your current health, including any conditions related to your time in Service. This will help your GP to give you the right treatment and, if necessary, refer you to specific services for those who have been in the military. Please see here for more information.
